Scheduling an Appointment
Patients are seen by appointment Monday through Friday, 9:00 a.m. to 4:30 p.m. Our receptionists are available daily until 5:00 p.m. to assist with your appointment scheduling. If you are not able to keep your appointment as scheduled or are going to be late, please contact our office as soon as possible.
New Patient Visits
Please bring the following to your first appointment:
- Your insurance card
- Completed registration forms (see below)
- A list of current prescription and/or over-the-counter medications you are taking (please include your dose and frequency)
- Pertinent information about your medical and surgical history
- Recent X-rays or other test results related to your condition
If you are unable to complete the registration information (below) prior to your visit, we request that you arrive at least 30 minutes prior to your scheduled appointment time to do so. However, we respectfully request that you print these forms, fill them out, and bring them with you:
- Registration Form
- Health History Questionnaire
- Telephone Policy
- Notice of Health Information Practices (Privacy Policy)
- Financial Policy

Emergency Care
Although our phones are answered 24 hours a day, if you have a true emergency, it is best to call 911 or go to the nearest Emergency Room for immediate treatmen
Fees
Fees for medical services provided vary and are determined by the complexity of the individual patient’s condition. If you have questions about fees for a particular service, one of our billing associates will be happy to discuss this information with you.
Insurance Information
Currently, Urology Associates of Northeast Florida accepts patients with most health plans.
Please call our office to verify that we still accept your insurance provider. Please also check your insurance plan's provider list to be sure you are able to be seen in our office
Please be aware that your insurance provider may require a referral letter from your primary care provider. It is the responsibility of the patient or guardian to pay any co-payments, and other charges not covered by your insurance provider. Please also review your coverage rules to determine if pre-authorization and second surgical opinions are applicable to you. Our staff will be happy to assist you; however, you are responsible for making this determination.
Billing Information
As a courtesy to our patients, we provide insurance billing for insurance plans we accept. Anything not covered by insurance will be your responsibility. We request payment at the time of your appointment for services that are not covered by insurance. Your insurance company may also require you to pay a co-payment at the time of your appointment. If there is a deductible on your insurance policy, you are responsible to make prompt payment if our office is not reimbursed for services rendered. Our staff will work with you regarding payment options, and in some cases, a payment plan can be instituted. Questions concerning billing, accepted insurances or professional fees will be happily discussed with you by our staff during office hours. Questions regarding your plan's coverage and benefits should be directed to your employer or insurance company.
Self Pay
Patients without insurance coverage will be responsible for all charges incurred. At the time of service, payment in full is expected and customary. Should you need financial assistance, we will work with you so that charges will not be a burden. Should you have any questions about fees or payment options, our staff will be happy to assist you.
Online Bill Payment
You can make payments to your account anytime, through our secure payments website: (payment website is currently offline, and we are working to fix it...12/31/2015) You will need your account number (which can be found on your statement) and a major credit card to make payments.
Referrals
If you have been referred to the us by another physician or have a family physician, please inform us. We will be able to keep your physician current on your diagnosis and condition. Please also remember to determine if pre-authorization is necessary prior to your appointment.
Confidentiality
Your medical record is strictly private. We will not give out information regarding your condition without your written authorization. The only exception to this is when required by law. You may request copies of your medical records for personal use, or for use by another physician by filling out our Medical Records Release Authorization Form
